The Anchorage Tribes of Tlingit & Haida Indians of Alaska’s 2022 Supplemental Rescue Small Business Grant application is to be completed by the applicant or an authorized representative. Failure to submit the information requested will result in a delay in processing and may result in a denial of the application.
All applications will be processed in the order received. You will receive an email confirmation when your application has been received.
- Photocopy of current Identification card and T&H Tribal Enrollment card
- Photocopy of each receipt submitting relief for up to $5,000.
- A signed and dated “Schedule of Expenses” matching the receipts submitted.
- Photocopy of Business License, Articles of Incorporation, or LLC document showing ownership.
- The Business Tax Identification Number requested (TIN, EIN, SSN) is the number that the business reports its income to the Internal Revenue Service.
Purpose of Funds/Data Collection
Please attach documents, receipts, lease agreements, contracts for each item you are seeking relief for your application. Please remember to upload your enrollment ID and business license.
